Privacy Notice
We are required by a Federal law called 'HIPAA' to inform you of the privacy practices used in our office to safeguard your personal health information. This notice describes how medical information about your may be used and disclosed. It also describes how you can access your medical information. A more detailed notice of our Privacy Practices is in our waiting room.
Your vision/health records at out office are confidential. We will not release or disclose this information to anyone but you, unless we obtain your permission. In addition your written permission will specify what information is to be released, to whom, and it will have an expiration date. There are exceptions to this rule, and the HIPAA law states that we must inform you of these exceptions. Please see the attached yellow paper for a list of exceptions.
If you have the right to review your paper or computer records in our office upon request. You may also request that amendments be made to your records. Another one of your rights is to request that amendments be made to your records. Another one of your rights is to request an accounting of disclosures of your records for the past 6 years.
